Mission Statement

The mission of Health Services of Lyon County, is to promote the public health and well-being of all residents. This is accomplished through a united commitment to assess individual and community needs and provide education and prevention services on health and wellness.

Health Services of Lyon County (HSLC) serves nearly 11,800 residents through various programs that promote healthy life choices, environmental and food safety, immunizations, and disease prevention. Our agency plays a prominent role with collaborating with our community and health partners to initiate innovative programs in order to address specific health issues.

Vaccination Clinics

Health Services of Lyon County offers vaccines to children who meet the qualifications of the VFC (Vaccines for Children) program and has the ability to bill the following insurance carriers: United Health Care (private), Blue Cross Blue Shield, Avera Health Plan, Sanford Health Plan, HAWK-I (United Health Care and AmeriGroup), and the following state health insurance plans: United Health Care and AmeriGroup.

Phase 1A - Active Now -Priority is to vaccinate all healthcare personnel, long term care staff and residents.

Phase 1B

COVID-19 Vaccine Lyon County Information

Public Health along with our community partners Avera and Sanford are moving to Tier1 of Phase 1B of vaccine planning.

Tier1-Phase1B includes the following:

  65 and older

  PK-12 schools, daycare social workers and    early childhood education

  First Responders

SCHOOLS - Public Health is working directly with the schools; school staff do not call Public Health.

DAYCARE/EARLY CHILDHOOD/SOCIAL WORKERS - Contact Public Health at 712-472-8200 if you are interested in the vaccine.

FIRST RESPONDERS - Public Health is working directly with all First Responder Groups.  Call Public Health with any questions at 712-472-8200.

PERSONS 65 AND OVER - Lyon County residents who see a doctor OUTSIDE of the county and have not already contacted our office, please call 712-472-8200 and we will get you on the list to receive the vaccine.

Lyon County residents who see a doctor with Avera or Sanford Rock Rapids, your provider will notify you directly.

IDPH continues to urge Iowans that while the vaccine is still a scarce resource, to practice virus mitigation efforts.

Phase 1C - Not active yet - No determination has been made.

Public Health will utilize all healthcare partners and media outlets to educate the public on when widespread vaccine is availalbe.
